I cut a rectangle in both the back and the insert to get some airflow. Includes the belt clip and a screen protector (not too scratched up). As for the US (and this screws up a lot of people) contiguous US is the Lower 48 states, all the the ones that touch each other. Continental US includes Alaska as it's on the North American continent. Don't know about Hawaii. sk
